Telehealth Disease Management Program

St. Vincent's Home Health Telehealth

St. Vincent’s Home Health program will feature telehealth monitoring. Equipment will be set up at patient’s homes so their vital signs can be monitored daily by critical care nurses working outside of their home. The nurses review the patient’s data, interact with them remotely and initiate appropriate clinical intervention in collaboration with Jacksonville-based home health nurses and staff. These patients are monitored every day in an effort to keep them healthy and reduce unnecessary hospital readmissions.

"This joint venture will help improve our alignment with the continuum of care for congestive heart failure patients.  We want to take care of our patients at all times, not just when they enter the hospital doors," said Steven Nauman, MD and Cardiologist with St. Vincent’s Cardiology.
