Heart Failure Education
St. Vincent’s Cardiology is your partner in heart disease. Our program is not solely focused on treatment. We want to arm you with knowledge on how to live with and or prevent future complications from heart disease. Our goal is to provide you with information on how to take better care of your heart so you don’t end up back at the hospital.
Our heart failure program is designed and implemented by a team of certified, multidisciplinary professionals who have extensive experience caring for heart failure patients in inpatient and outpatient settings. After a heart attack, you’ll be guided throughout your hospitalization and the transition to home by this team of experts, starting in our Heart Failure Clinic.
Heart Failure Clinic
Within the first week after you’ve been discharged from the hospital, you’ll attend your first 30-minute session at our Heart Failure Clinic. During this session, an advanced registered nurse practitioner will review your current recovery regimen and optimize it to ensure it works best for you. The clinic is available two afternoons a week.
Heart Failure University
In addition to the Heart Failure Clinic, we offer a free program designed to help you increase your understanding of heart failure and learn ways to manage the disease. Through eight classes over four weeks, you’ll come to learn how to live with heart failure. Classes include:
- The Anatomy and Functions of the Heart
- Heart Failure Medications
- Advanced Care Planning
- Stress Management
- Treatment Options
- Physical/Occupational Therapy and Exercise Recommendations
- Lifestyle Changes
Classes are 1:00 p.m. – 3:00 p.m. at St. Vincent’s Riverside on Tuesdays and Thursdays, and are available to all heart failure patients and their families. Each class is one hour of education and one hour of supervised exercise.
