The St.Vincent’s HealthWorks program partners with Northeast Florida employers to provide health management solutions for their employees.  We provide services to meet the needs of all organizations, regardless of company size, or industry.

As a health system, we integrate a traditional health promotion program with clinical experts and by doing so are able to measure the success and financial impact of the programs and services we implement.  Our approach to employee health management is strategic.  This ensures that initiatives are results-oriented, and focused on improving the health and lowering cost of care for the organizations we partner with.

 

On-Site clinics.

On-site clinics no longer exist just to treat people who are injured on the job, they offer a way for employers to improve employee health and contain healthcare costs. Our Clinics not only treat sore throats, sinusitis and runny noses, but also help employees (and dependents) stay healthy.

Our St. Vincent’s HealthWorks On-Site Clinics are customized to meet the needs of individual employers.  They can be operational either part or full time and provide primary care, with an emphasis on preventive screenings, wellness and chronic care management.  

Convenient access to care and longer appointments give your employees an opportunity to not only better understand their health risk, but to establish trusting relationships with our with our HealthWorks clinicians.

Onsite Health Screenings

We can offer a wide range of onsite screenings for employers, from popular fingerstick biometrics (Cholesterol, HDL/LDL, Glucose), blood pressure and BMI, to more comprehensive panels that include PSA, creatinine etc. We can customize screenings to meet the need of any employer.

Screening services do include aggregate data for the employer, and a comprehensive individual report for the participants.

Personal Health Profiles

Worksite WellnessAvailable online or on paper, these profiles allow us to gather behavioral risk data for your population, Survey asks questions about smoking, preventive care habits, as well as addressing exercise, stress and current risk factors.  

Aggregate reporting is provided back to employers.  The data is essential when it comes to managing the health of a population. The Personal Health Profile, is most effective when combined with our Clinic or On-Site Screening Service.

Wellness Programming

Based on data collected by population, the St.Vincent’s HealthWorks Team will make recommendations and may facilitate some clinically based onsite programs to ensure conditions are managed (eg. diabetes) and to further manage risk and coach your employees with their wellness goals.

Health Information Centers (HIC)

The health Information Center is our center for communicating pertinent health issues. Each month this movie-size poster presents a new wellness topic to encourage your employees to live healthier.  Pamphlets with four wellness topics are rotated quarterly and are included with the HIC for your employees to have additional education at their fingertips.

Online Wellness Portal for Employers.

OneCommunity is our online wellness portal.  Linked to unique an innovative set of resources and tools, this portal will help you track and motivate your employees while allowing you freedoms to customize your program.  One Community Features:
