Guests Always Welcome—Each of the 10 private rooms at the Community Hospice Center for Caring at St. Vincent’s Riverside has been designed with comfort in mind for patients and their loved ones. Guests can visit any time day or night, staying to sleep, if they wish, in reclining chairs or sofas.

The sixth and newest inpatient care center of Community Hospice of Northeast Florida is a specialized, homelike facility designed to serve the short-term, acute care needs of hospice patients in our community.

The nearly 6,500 sq. ft. of space includes 10 private patient rooms, as well as comforting, homelike amenities, a family lounge and a kitchenette for use by family and visitors. It’s part of the Community Hospice commitment to extend the continuum of care in a faith-based acute care setting to meet patients’ end-of-life needs.

Conveniently located on the fourth floor of St. Vincent’s Riverside campus, the Community Hospice Center for Caring ensures ready access for patients already receiving services from the hospital’s medical staff and for those nearby.

Community Hospice and St. Vincent’s HealthCare have a long history together. Community Hospice works with St. Vincent’s staff to make hospice services available to their patients—with on-site admissions staff at their Riverside and Southside locations, as well as through patient care provided at their skilled nursing facility, St. Catherine Labouré Manor.

Care for Families, Too—The unit includes a spacious family lounge for loved ones to relax and spend time together, with books, cards, games and Internet access.

Community Hospice patients and their families receive care from an interdisciplinary team of hospice professionals dedicated to their comfort. They focus on meeting individual needs 24-hours a day, seven days a week in this unique setting. Team members include a physician or advanced registered nurse practitioner, nurse, psychosocial specialist, certified nursing assistant and, as requested, chaplain, volunteer and bereavement expert.
