The St. Vincent’s Home Health program allows our Ministry to better serve patients that need care after their discharge from the hospital or for those that need assistance managing chronic disease conditions. Home health programs improve patient care after hospital discharges and reduce hospital readmission rates.

Telehealth Disease Management Program

St. Vincent’s Home Health program will feature telehealth monitoring. Equipment will be set up at patient’s homes so their vital signs can be monitored daily by critical care nurses working outside of their home. The nurses review the patient’s data, interact with them remotely and initiate appropriate clinical intervention in collaboration with Jacksonville-based home health nurses and staff. These patients are monitored every day in an effort to keep them healthy and reduce unnecessary hospital readmissions.
