High Reliability Organization (HRO)


High Reliability Organization It goes without saying that people want to come to a hospital that is safe. St. Vincent’s is taking patient safety to another level. We, along with our parent organization Ascension Health are striving to become a High Reliability Organization (HRO). According to the Agency for Healthcare Research and Quality (AHRQ), HROs are organizations with systems in place that are exceptionally consistent in accomplishing their goals and avoiding potentially catastrophic errors. Ascension Health expects its hospitals to Heal without Harm by 2014. In order to accomplish this, each Ascension hospital across the country holds something called a “Safety Huddle” each morning. Leaders throughout the organization gather to openly discuss failures and “near misses”. Ascension wants its hospitals to create a blame-free environment to support the reporting of errors so we may better understand them. The idea is that “to err is human” but through analysis and openness we can create a supportive culture that learns from its mistakes so we do not repeat them.
