Obtaining Your Medical Records

If you would like a copy of your medical records, please contact the Medical Records Office at the hospital that treated you. St. Vincent's HealthCare keeps patient medical records up to a maximum of 10 years.

For individuals seeking copies of their records, the cost is $1 per page. Record requests from other treating physicians or hospitals are complimentary. 

You can make a medical records request by phone, online or in person.

To protect your medical and personal information, your signature is required to obtain your medical records. Please fill out a Release of Medical Information Form and either fax or mail it to the healthcare facility where you were treated.

In Person

In order to receive a copy of your records, you must present a valid picture ID or Power of Attorney documentation. You may pick up your records at our records offices between 8:00 a.m. and 4:30 p.m., Monday through Friday, at the following locations: 

  • St. Vincent's Medical Center Office - Second floor of the main building
  • St. Luke's Hospital Office - Ground floor of main building

How To Obtain X-Rays And Other Images

You may contact the Imaging Offices at St. Vincent's Medical Center or St. Luke's Hospital to obtain copies of your X-rays, mammograms and other imaging results. These results are offered on CD at no charge. In order to receive a copy, you must contact the facility where you were treated at least 24 hours in advance.
