St. Vincent’s HealthCare & Family Care Partners Create Program to Improve Coordinated Care for Patients
St. Vincent’s HealthCare, a ministry of Ascension, the nation’s largest non-profit health system, and Family Care Partners are creating a partnership to provide higher levels of patient-centered care coordination between St. Vincent’s hospitals and all Family Care Partners locations.
The goal of the program is to make the transition between hospital and outpatient care settings easier for patients. Family Care Partners will provide hospital staff with an electronic copy of a patient’s up-to-date outpatient care record to help ease these transitions without losing key clinical information.
“Our goal is to successfully improve the quality and safety of the care we deliver, improve the experience for patients and physicians and to reduce the overall cost of care,” said Kyle Sanders, President of St. Vincent’s Southside and System VP of Population Health Management. “We hope this clinically integrated network with Family Care Partners will be a model that delivers exceptional care to our community.”

Family Care Partners has eight locations in Jacksonville and serves thousands of patients.
